The use, possession, or being under the influence of illegal drugs or alcohol while on Company time is prohibited and will result in disciplinary action up to and including discharge. If offered a position, a pre-placement drug test would be required in addition to any subsequent physical examinations at the company's sole discretion. Refusal to submit to such testing during the course of employment may result in disciplinary action, up to and including discharge.
An applicant for employment with a sealed record on file with the commissioner of probation may answer "no record" with respect to an inquiry herein relative to prior arrests, criminal court appearances or convictions. In addition, any applicant for employment may answer "no record" with respect to any inquiry relative to prior arrests, court appearances and adjudications in all cases of delinquency or as a child in need of services which did not result in a complaint transferred to the superior court of criminal prosecution. The attorney general may enforce the provisions of this paragraph by a suit in equity commenced in the superior court.
I authorize investigation of all statements contained in this application if I am considered for employment. I also authorize previous employers named, or any other person or persons to whom you may refer, to give any and all information regarding my employment or scholastic standing together with any other pertinent information and I release all persons from any liability or action based upon such verifications.
I understand I will be required to take a physical examination after an offer of employment is extended to me but before I actually commence work. I authorize the examining physician to release findings. I understand that misrepresentation or omission of facts requested, the receipt of unsatisfactory references, an unacceptable CORI report, or failure to pass the prescribed physical examination will be cause for withdrawal of the employment offer or dismissal from my employment, if I have already commenced work.
I further understand that if I am employed, my employment is not contractual in nature and is for no set period of time. It may be terminated by me or by my employer at any time. If employed, I agree to abide by all rules and regulations. I understand the rules, regulations, procedures and benefits are subject to change at any time. No statement, whether oral or written, by any representative of one facility, with the exception of a written statement by the President, can vary any of the terms and conditions of my employment.
